(4)根本原因分析 ROOT CAUSES
发生原因CAUSE(S)  FOR OCCURRENCE:
Vendor wanted to use a higher power rating resistor such that it can withstand higher current as well as temperature but they have not go through a proper approval procedure before implementing to the change.
(5)纠正措施CORRECTIVE ACTIONS
临时措施Interim Actions:
"1. Return all the sorted PCBA for vendor to rework.
2. All the inventory ready in vendor's warehouse required to be 100% reworked with the original low power rating resistor.
3. IQC will temporary add the visual check point for this resistor for incoming inspection in order to ensure the coming PCBA's delivery are soldered with the correct resistors. This visual procedure will be continue until the completion of current 80,000pcs order and will not be required anymore if the sample inspection result from the 80,000pcs prove this problem to be under control. "
永久措施Permanent Actions:
Request vendor to use the orginial lower rating resister for future production until any proposal changes approved by engineering department.
(7)预防 PREVENTION
预防措施Preventative Measures:
QC department is working with their team to see if it is possible add a procedure to random audit the incoming material in order to ensure all vendors are following the original material specification or approved sample if necessary.
